OSHA statistics indicate that there are roughly 85 forklift fatalities and 34,900 serious injuries each year, with 42 percent of the forklift fatalities from the operator's being crushed by a tipping vehicle.

 

When a forklift is "out of level" it can easily tip in any direction. 

According to transportation loss and damage claims experts, the most common cause of load damage is forklifts. One reason is because there is pressure on the drivers for production, so they often do their jobs too quickly, which leads to damage. Even if they have received training on how to operate forklifts safely, they may work too quickly thus bypassing safe operations.

Even after training, experience is what makes an operator more successful.  The most difficult area to master is the tilt function of the forklift.  In many cases, the operator will still struggle with proper tilt handling especially in elevated positions.  The higher the mast goes, the harder it is to achieve alignment.
